The Church of God Benefits Board All the forms needed to apply for a loan with the Benefits Board are available here. Each form listed below should be completed in its entirety. The completed forms, along with the $100 application fee and a real estate appraisal of all church owned property, should be submitted to the Board by mail to the following address:
